Case Manager

The Champaign-Urbana Public Health District (CUPHD) is seeking a full-time (40-hour week) Case Manager for the Family Case Management program in the Maternal and Child Health Management division.Family Case Managers provide help and support to income-eligible pregnant women and babies.

Responsibilities include:

  • Access family needs, plan services, make referrals
  • Provide health education
  • Conduct home visits and advocate for clients
  • Conduct developmental and maternal depression screenings

Requirements include:

  • A bachelor's degree in social work, sociology, psychology, human services, or a closely related field and one year of professional social work, counseling, or closely related experience is required
  • Or an associates degree in social work, sociology, psychology, human services or a closely related field and two years of professional social work, counseling, or closely related experience is required.
  • A valid driver's license, dependable transportation, and proof of auto insurance
  • Bilingual is highly recommended. CUPHD offers qualified bilingual employees' additional compensation once approved

The starting annual salary for a Case Manager is $50,515 for a 40-hour week.
