| Description |
PURPOSE OF POSITION
RN needed to care for medically fragile children 1:1 in the home setting. Nurse needs to have pediatric experience including ventilator, tracheostomy and G-tube care. Prior experience in home care and home care documentation preferred. Candidate should have flexible availability including every-other weekend and be able to work on more than one case.
1. Independently provides direct care for pediatric client in their home following plan of care. 2. Administers medications and treatments as prescribed by physician. 3. Communicates effectively with all team members including client, family, co-workers. 4. Completes all required documentation in a timely manner. 5. Perform other related duties as assigned by supervisor.
QUALIFICATIONS
This position requires:
1. Education: LPN or RN (BSN preferred), Current licensure in state of Illinois. 2. CPR certification and valid driver's license. 3. Necessary experience: one year of home health experience :trachs, G-tube feedings. Pediatric infusions and ventilator experience a huge plus. 4. Communication: ability to read and write English, good interpersonal skills, team player, good judgment and organization. 5. Excellent observation, problem-solving skills, math skills, and nursing skills. 6. Genuine compassion for the ill and disabled, project a friendly personality, understand customer service and team building concepts, ability to deal with clients in a pleasant and professional manner is required. 7. Ability needed to work independently, ability to follow through on assigned tasks, demonstrate initiative.
