Position Summary:
With specific direction and routine supervision from the Director of Community Health Systems, the CCU Care Coordinator performs Community Care Program case management services which includes professional assessments of the physical, social and economic needs of individual program applicants and/or clients and determines financial eligibility for these services, as well as, the "Choices for Care" client screenings for possible long term placement and the determination of need and eligibility for client financial management services provided a local community based senior services agency. This position is a professional job that requires the employee to exercise appropriate discretion and judgement in the performance of his/her assigned duties and responsibilities.
Qualifications:
EDUCATIONAL REQUIREMENTS
Bachelor's Degree in Social Work, Social Science or related field or College Diploma in Nursing plus one year of program experience or Certificate in Practical Nursing plus one year of program experience
CERTIFICATION & LICENSURE REQUIREMENTS
Registered Professional Nurse (RN) License Illinois, if degree is in Nursing upon hire or Licensed Practical Nurse (LPN) Illinois, if degree is in Practical Nursing upon hire and Driver's License upon hire and Proof of Auto Insurance upon hire.

Requires the ability to respect and adapt readily to a diverse variety of complicated patient care situations. Requires the employee to recognize and respect the confidentiality of all client or patient records, as well as the confidentiality and/or privacy of co-worker's records.Requires the delivery of all services in a tolerant and consistent manner.
