Position Summary:
This person is responsible for overseeing pharmacy technicians and providing pharmaceutical care according to the rules and regulations of the State of Illinois and according to the policies and procedures of the pharmacy department.

Duties and Responsibilities
  • Practice pharmacy according policies and procedures of the Pharmacy Department, the Practice Standards of the American Society of Hospital Pharmacists and the State of Illinois Pharmacy Practice Act.
  • Abide by all state and federal laws concerning the practice of pharmacy.
  • Maintain accurate patient profiles which contain a minimum of the following information:  patient name, room number, identification number(s), allergy information, current and applicable diagnosis information, weight, age, sex, physician(s), date of admission; as well as a record of all current and discontinued medication orders for the patient.
  • Verify all Computerized Physician Order Entry (CPOE) orders received in the pharmacist queue in the Electronic Medical Record (EMR) prior to initial dispensing.  Review will include, but is not limited to:   contraindications, allergic reactions, drug-drug interactions, food-drug interactions, incompatibilities and/or therapeutic duplication.  This includes remote order entry as required.
  • Enter orders into the EMR as needed.
  • Maintain the EMR system for the nurses so medications can be administered as safely as possible. This includes ensuring medications are barcoded and ready for scanning by the nurse at time of administration.
  • Accurately dispense drug products needed to fill all pharmacy orders, not loaded in an Automated Dispensing Machine (ADM), on a daily basis.
  • Maintain the unit dose system in accordance with the current policy and procedure of the department.
  • Ability to aseptically prepare small and large volume parenteral solutions and TPN’s to allow dispensing in a timely and cost effective manner.  Must also verify and sign off on all parenteral products prepared by a pharmacy technician.
  • Monitor daily usage of DEA Class II - V controlled substances and accurately document quantities dispensed to fill ADM’s and ensure an adequate supply of these drugs on each nursing unit to meet the current needs of the patient population.
  • Take responsibility for and verify all pharmacy technician work prior to the final dispensing of medications.
  • Dispense only those drug products listed in the current drug formulary of Rochelle Community Hospital (RCH) when possible according to policy and procedure.
  • Monitor all patient drug regimens on a daily basis for any complications listed above in number 4.  If necessary, consult with physicians, nurses, and other members of the health care team in regards to issues that may arise and affect patient care and/or personal, departmental or hospital liability.  Document such interventions in the EMR and their results according to current policy and procedure.
  • Maintain the automated dispensing machines (ADM’s), in collaboration with the pharmacy technician to ensure adequate supplies of medications are available.
  • Counsel patients about the medications they receive so that they are aware of the names, descriptions, doses, administration techniques, major side effects and interactions; and are thus able to obtain the maximum benefit from their medication regimen.
  • Utilize available resources in order to provide drug information to physicians, nurses, and other members of the health care team as well as the public as requested.
  • Order, receive and stock medications as necessary.
  • Restock and seal open crash carts. 
  • Oversee and/or assist with monthly inspections of all drug stock in the hospital in accordance with policy and procedure. 
  • Inspections include but are not limited to confirming cleanliness of area, making sure it is organized, and checking for outdates.
  • Accurately comply with all documentation required by the policies and procedures of the department.
  • Assist the Director of Pharmacy in gathering data needed for the departmental Performance Improvement Plan as requested.
  • Stay up to date with departmental and hospital wide developments by reading all new items disseminated to staff via e-mail, educational programs, according to policy and procedure.  This may include verbal notice as well.
  • Maintain professional competency by completing periodic departmental staff skills and development programs as directed. 
  • Maintain mandatory educational requirements based on JCAHO, Illinois Board of Pharmacy, IDPH and OSHA standards including at least the minimum amount of continuing education credits as required by the State of Illinois Pharmacy Practice Act. 
  • Attend all mandatory hospital in-services and document all activities.
  • Responsible for the proper functioning of the departments in the absence of the Director of Pharmacy.
Education
  • Minimum of B.S. in Pharmacy and a license to practice Pharmacy in the State of Illinois.
Experience
  • Experience as a hospital Pharmacist is preferred but not required.
  • Excellent communication and organization skills are required.
  • Ability to operate computers.
  • Ability to interact with physicians, patients, and employees is required.
Work Environment/Physical Demands
  • The ideal candidate must be able to work and complete all physical requirements of the job with or without a reasonable accommodation.
  • Demonstrates the ability to work accurately and effectively in oral and written directions and communication.
  • Enjoys working with people and works cooperatively with other hospital staff.
  • Medium Lifting, generally lifting not more than 50 lbs. maximum with assistance, with frequent lifting and/or carrying of objects weighing up to 25 lbs.
